Mining Fatigue is a negative status effect that slows how fast you break blocks and how fast you swing your weapon. If you have ever swum into an ocean monument and watched a single block of prismarine take the better part of a minute to break, you have met it.
Unlike most effects, you cannot brew Mining Fatigue in survival. There is no potion for it. Almost every time a player runs into it, an elder guardian put it there on purpose, as a way to stop you from tunneling straight to the gold hidden inside a monument.

What is Mining Fatigue?
Mining Fatigue is a status effect with the ID minecraft:mining_fatigue. Its icon is a dull brown pickaxe-style symbol in your inventory, and while it is active you will see faint grey particles drifting off your character.
The effect does two things: it drops your mining speed and it drops your attack speed. It does not touch your movement, so you can still walk and swim at normal pace. That is the difference between Mining Fatigue and Slowness. Slowness is about your feet. Mining Fatigue is about your hands.

Where Mining Fatigue comes from

Elder guardians are the large, spiky boss-style mobs that live in ocean monuments. Each monument has three of them. When you swim near one, it casts a curse: your screen flashes with a ghostly image of the guardian, a low sound plays, and you get Mining Fatigue III for five minutes.
The catch is that the guardians re-cast the curse every so often as long as one is alive and can sense you. So you cannot just wait it out inside the monument. The clock keeps resetting. Commands
If you are testing something or building a map, you can give yourself the effect directly with /effect give @s minecraft:mining_fatigue. Remember that the amplifier is zero-based, so amplifier 2 is level III.
What Mining Fatigue actually does
The effect multiplies your mining speed by a small number. Lower is worse. Here is how the levels compare:

Level III is the one that matters, since that is what you get in a monument. At about 0.27% of your normal speed, a block that would pop in half a second instead drags on for the better part of three minutes. Prismarine is already a slow block to mine, so the combination feels close to impossible.
The same slowdown applies to your attack speed. Your sword swings recharge far slower, so the white attack-strength bar takes much longer to fill back up. That is another reason the guardians are dangerous while the curse is up. You are slow to break blocks and slow to hit back at the same time.
One thing worth knowing: Haste does not save you here. Haste speeds mining up, and the game does apply both effects together, but Haste II only adds a modest boost. Against the huge penalty of Mining Fatigue III it barely registers. A beacon with Haste helps a little after the curse is gone, not while it is active.
The effect also does nothing to explosions, which is the loophole most players miss. Mining Fatigue only slows the blocks you break by hand or with a tool. TNT, beds in the Nether, and other blasts break blocks on their own timer, so a well-placed charge can tear through prismarine even while you are fully cursed.
How to get rid of Mining Fatigue
You have three real options, and which one to use depends on where you are.
Drink milk
A bucket of milk clears every status effect you are carrying, Mining Fatigue included. Outside a monument, this is instant and free if you have a cow nearby. Inside a monument it is a trap, because the moment you drink it a living elder guardian will just curse you again on its next cast. Milk only sticks once the guardians are dead.
Kill the elder guardians
This is the proper fix inside a monument. There are three guardians, and once all three are dead nothing is left to re-apply the curse. Your current Mining Fatigue will still tick down its five minutes, but you can end it early with milk now that it will not come back. After that you can mine the prismarine and dig out the gold at full speed.
Wait it out
If nothing is re-applying the effect, it simply expires. Five minutes from an elder guardian, or whatever duration a command set. This is the slow option, but if you are far from a cow and no guardians are chasing you, it works fine.
Mining out a monument with the curse active
Most players run into Mining Fatigue for the first time while raiding an ocean monument, so a few practical notes help.
The gold is worth going for. Each monument holds eight gold blocks tucked inside a chunk of dark prismarine near the center. That is 72 gold ingots once you break them down, which is more gold than you will find in a long mining session.
Because the curse makes mining pointless, deal with the guardians first. Bring good armor, a strong sword, and ideally Respiration and Aqua Affinity on your helmet so you can see and mine underwater without penalty later. Potions of Night Vision and Water Breathing make the whole trip far easier. Some players drain sections of the monument with sponges or sand first so they can fight and mine in air pockets.
Once the three guardians are down, clear the curse with milk or wait for it to expire, then mine straight to the gold. The regular guardians that still spawn are a nuisance, but they cannot cast Mining Fatigue. Only the elder guardians do that.
Tips and things players get wrong
A few practical points save a lot of frustration on your first monument raid.
Time your milk carefully. A bucket of milk strips every effect at once, not just Mining Fatigue. If you drink it while you still have Night Vision and Water Breathing running, you lose those too. Save the milk for after the guardians are dead, and re-drink your good potions afterward if you need them.
The curse reaches you through walls. An elder guardian does not need a clear line of sight to cast it, and its range covers a large part of the monument. Ducking behind a prismarine wall will not protect you, so do not count on hiding.
Enchantments on your tools still help once the curse is gone. Efficiency, plus Aqua Affinity on your helmet so you mine underwater at full speed, plus Respiration so you can stay down longer, all pay off during the actual gold dig. None of them fight Mining Fatigue itself, but they make the rest of the trip quick.
Finally, do not panic when the ghostly guardian flashes on your screen. The jump-scare animation and sound are just the curse being applied. They do no damage on their own.
